FSA and HSA cards are always welcome.

At this time, the clinic only accepts United Healthcare and UMR insurance only. If you plan to use your United or UMR benefits, please verify the following with your insurance provider prior to your visit:

• Number of treatments allowed per year

• Co-pay amount

• Whether your visits are subject to a deductible

For clients with other insurance carriers, a superbill with ICD-9 codes can be provided upon request for potential reimbursement. If you plan to seek reimbursement, it is best to confirm coverage with your insurance company before beginning treatment.

United and UMR Infromation on Cupping/Massage Services

Beginning in the new year, there will be a change to 90-minute sessions that include cupping. United Healthcare and UMR do not cover cupping, so clients who choose to include cupping in their treatment will be responsible for an additional out-of-pocket fee.

Session coverage details:

• 60-minute sessions are covered by United Healthcare and UMR and require only the standard co-pay.

• 90-minute sessions with cupping include an additional $25 out-of-pocket fee.
